hv: replace CPU_PAGE_SHIFT with PAGE_SHIFT

- replace CPU_PAGE_SHIFT with PAGE_SHIFT
  These two MACROs are duplicated and PAGE_SHIFT is a more
  reasonable name.
- remove unused MACROs related to page shift in cpu.h
