Rename some C preprocessor macros:
  NUM_GUEST_MSRS --> NUM_EMULATED_MSRS
  CAT_MSR_START_INDEX --> FLEXIBLE_MSR_INDEX
  NUM_VCAT_MSRS --> NUM_CAT_MSRS
  NUM_VCAT_L2_MSRS --> NUM_CAT_L2_MSRS
  NUM_VCAT_L3_MSRS --> NUM_CAT_L3_MSRS
