hv:fix crash issue when handling HC_NOTIFY_REQUEST_FINISH

Input 'vcpu_id‘ shall be validated properly:
- 'vcpu_id' shall be less than 'vm->hw.created_vcpus'
instead of 'MAX_VCPUS_PER_VM'.
