The break key (key value 0x0) was used as switch key from guest serial
to hv console and guest serial could not receive break key. This blocked
some guest debugging features like KGDB/KDB, sysrq, etc.
This patch leverages escape sequence "<escape> + <break>" to send break to
guest and "<escape> + e" to switch from guest serial to hv console.

Many of the license and Intel copyright headers include the "All rights
reserved" string. It is not relevant in the context of the BSD-3-Clause
license that the code is released under. This patch removes those strings
throughout the code (hypervisor, devicemodel and misc).

support pci-vuart type, and refine:
1.Rename init_vuart() to init_legacy_vuarts(), only init PIO type.
2.Rename deinit_vuart() to deinit_legacy_vuarts(), only deinit PIO type.
3.Move io handler code out of setup_vuart(), into init_legacy_vuarts()
4.add init_pci_vuart(), deinit_pci_vuart, for one pci vuart vdev.
and some change from requirement:
1.Increase MAX_VUART_NUM_PER_VM to 8.

The vuart_read()/vuart_write() are coupled with PIO vuart type. Move
the non-type related code into vuart_read_reg()/vuart_write_reg(), so
that we can re-use them to handle MMIO request of pci-vuart type.

As ACRN prepares to support platforms with multiple IO-APICs,
GSI is a better way to represent physical and virtual INTx interrupt
source.
1) This patch replaces usage of "pin" with "gsi" whereever applicable
across the modules.
2) PIC pin to gsi is trickier and needs to consider the usage of
"Interrupt Source Override" structure in ACPI for the corresponding VM.

Remove debug command 'vuart' which is used to change VM's console's base
and irq before bringing up with uefi.
But after vuart's resuffle, the gloable variables vuart_com_irq and
vuart_com_base is not used anymore, vuart gets the base and irq from
its configuration. So the command can not work.

As there is a requirement for conmunication between VMs on uart port,
but for Pre-launched VM, the only way is to create vuart for each VM and
connect them at hypervisor side. So, the vuart code should be moved from
/debug to /dm so that they can also be used in release version.
For the console related code, as no need to support in release version,
so leave them at debug directory.
