Convert IRQ-related macros to static inline functions and introduce
wrappers for arch-specific implementations. This follows the style we
defined for multi-arch development.
This is a follow-up update for commit
a7239d126 ("[FIXME] hv: risc-v add denpended implementation in cpu.h").

Extract common interface to include/lib/bits.h, and invoke the variant
implementation of arch.
Re-implement unlocked functions as C in common library.
Rename bitmap*lock() to bitmap*(), bitmap*nolock() to bitmap*non_atomic().

Move x86 architecture dependent per cpu data into a
seperate structure and embeded it in per_cpu_region.
caller could access architecture dependent member by
using prefix 'arch.'.

Many of the license and Intel copyright headers include the "All rights
reserved" string. It is not relevant in the context of the BSD-3-Clause
license that the code is released under. This patch removes those strings
throughout the code (hypervisor, devicemodel and misc).

This patch adds an option CONFIG_KEEP_IRQ_DISABLED to hv (default n) and
config-tool so that when this option is 'y', all interrupts in hv root
mode will be permanently disabled.
With this option to be 'y', all interrupts received in root mode will be
handled in external interrupt vmexit after next VM entry. The postpone
latency is negligible. This new configuration is a requirement from x86
TEE's secure/non-secure interrupt flow support. Many race conditions can be
avoided when keeping IRQ off.

The common irq file is responsible for managing the central
irq_desc data structure and provides the following APIs for
host interrupt handling.
- init_interrupt()
- reserve_irq_num()
- request_irq()
- free_irq()
- set_irq_trigger_mode()
- do_irq()
API prototypes, constant and data structures belonging to common
interrupt handling are all moved into include/common/irq.h.
Conversely, the following arch specific APIs are added which are
called from the common code at various points:
- init_irq_descs_arch()
- setup_irqs_arch()
- init_interrupt_arch()
- free_irq_arch()
- request_irq_arch()
- pre_irq_arch()
- post_irq_arch()
