When LAPIC timer is working in oneshot or periodic mode, OS uses
initial counter register/current counter register to program
a timer. Both initial counter and current counter depend on the
LAPIC frequency. ACRN emulated vLAPIC timer based on the TSC.
vLAPIC freq is the same as TSC freq.

TLFS defined 2 vMSRs which can be used by Windows guest to get the
TSC/APIC frequencies from hypervisor. This patch adds the support
of HV_X64_MSR_TSC_FREQUENCY/HV_X64_MSR_APIC_FREQUENCY vMSRS whose
availability is exposed by CPUID.0x40000003:EAX[bit11] and EDX[bit8].

Many of the license and Intel copyright headers include the "All rights
reserved" string. It is not relevant in the context of the BSD-3-Clause
license that the code is released under. This patch removes those strings
throughout the code (hypervisor, devicemodel and misc).

Generalize and split basic cpu cycle/tick routines from x86/timer:
- Instead of rdstc(), use cpu_ticks() in generic code.
- Instead of get_tsc_khz(), use cpu_tickrate() in generic code.
- Include "common/ticks.h" instead of "x86/timer.h" in generic code.
- CYCLES_PER_MS is renamed to TICKS_PER_MS.
The x86 specific API rdstc() and get_tsc_khz(), as well as TSC_PER_MS
are still available in arch/x86/tsc.h but only for x86 specific usage.

TLFS spec defines that when a VM is created, the value of
HV_X64_MSR_TIME_REF_COUNT is set to zero. Now tsc_offset is not
supported properly, so guest get a drifted reference time.
This patch implements tsc_offset. tsc_scale and tsc_offset
are calculated when a VM is launched and are saved in
struct acrn_hyperv of struct acrn_vm.

TLFS spec defines that HV_X64_MSR_VP_INDEX and HV_X64_MSR_TIME_REF_COUNT
are read-only MSRs. Any attempt to write to them results in a #GP fault.
Fix the issue by returning error in handler hyperv_wrmsr() of MSRs
HV_X64_MSR_VP_INDEX/HV_X64_MSR_TIME_REF_COUNT emulation.

TLFS spec defines different hypercall ABIs for X86 and x64. Currently
x64 hypercall interface is not supported well.
Setup the hypercall interface page according to the vcpu mode.

In current architecutre, the maximum vCPUs number per VM could not
exceed the pCPUs number. Given the MAX_PCPU_NUM macro is provided
in board configurations, so remove the MAX_VCPUS_PER_VM from Kconfig
and add a macro of MAX_VCPUS_PER_VM to reference MAX_PCPU_NUM directly.

Two time related synthetic MSRs are implemented in this patch. Both of
them are partition wide MSR.
- HV_X64_MSR_TIME_REF_COUNT is read only and it is used to return the
partition's reference counter value in 100ns units.
- HV_X64_MSR_REFERENCE_TSC is used to set/get the reference TSC page,
a sequence number, an offset and a multiplier are defined in this
page by hypervisor and guest OS can use them to calculate the
normalized reference time since partition creation, in 100ns units.
