Most of the individual controllers were already converted earlier. Some log
calls were missed or added and then not updated during a rebase. Some of those
get updated here to fill those gaps.
Adding of the name to the logger used by each controller gets
consolidated in this commit. By using the name under which the
controller is registered we ensure that the names in the log
are consistent.
FeatureGate acts as a secondary switch to disable cloud-controller loops
in KCM, Kubelet and KAPI.
Provide comprehensive logging information to users, so they will be
guided in adoption of out-of-tree cloud provider implementation.
- Move from the old github.com/golang/glog to k8s.io/klog
- klog as explicit InitFlags() so we add them as necessary
- we update the other repositories that we vendor that made a similar
change from glog to klog

Add a separate method in a new file for creating cloud providers.
Currently the code is all mixed into the controller manager. We
should actively control what is made available to the cloud provider
so list explicitly the parms needed and move the code out. This will
avoid linkages to sneak in as we will catch it better during reviews.
