kubeadm: more random tokens

The strategy of hex encoding a random byte array only uses the
following characters:

	0123456789abcdef

Instead of the entire bootstrapping token character set:

	0123456789abcdefghijklmnopqrstuvwxyz

Update the token generation to use the entire character set. This
increases the token secret from 48 bits of entropy to ~82 bits.

256^8 (1.8e+19) vs. 36^16 (7.9e+24).

Fix exists status for azure GetLoadBalancer

We see a lot of log indicating load balancer not found in azure:

It's interesting that those service does not need loadbalancer, and caller is just checking whether one loadbalancer exists.
009701f181/pkg/controller/service/service_controller.go (L287)

And in we can see when err is not nil, it will not check exists value. Thus we should not return error when exists=false.

This was changed in:
edfb2ad552 (diff-c901394068476b4ccb003a6c6efad57cR63)

The PR removes the error when exists=false.

Fix vm cache in concurrent case in azure_util.go

Fix a bug in azure vm cache. In case two callers call 'getVirtualMachine', if the second caller sees non-empty request.VM, it should return that value, instead of the default 'vm' variable.

Add support for cloud-controller-manager in local-up-cluster.sh

**What this PR does / why we need it**:
We need an easy way to test the new external cloud provider. So
let's keep the existing CLOUD_PROVIDER and CLOUD_CONFIG as-is and
add a flag EXTERNAL_CLOUD_PROVIDER to run a separate process.

Since we use hyperkube to run controller-manager, let's add support
for cloud-controller-manager as well to support this use case.

  conflict ports, and it's expensive to do encoding/decoding
- Not to parse the container ports information in predicate meta, use

  addresses
- Collect scattered source code into common place

e21ecc0166 Updated Flexvolume setup mechanisms for COS instance image.
- If REMOUNT_VOLUME_PLUGIN_DIR is set to true, VOLUME_PLUGIN_DIR is remounted with `exec` option during cluster startup. This allows any writable location to be used as the plugin directory.
- New HostPath added to controller-manager deployment to enable access to volume plugin directory.
- Improved how the default directory is passed to master and node setup.

Fix `kubeadm upgrade` error while CoreDNS is installed

Given the node e2e Conformance tests over containerized ``Kubelet`` are already running and are published at https://k8s-testgrid.appspot.com/sig-node-kubelet#kubelet-containerized-conformance-aws-e2e-rhel, we can remove all mentions of the "experimental" keyword.

APIServices status when related service or endpoint change. So if APIServiceRegistrationController does not watch services, it will not miss any event for APIServices.
