Zide Chen 58ab26ea38 hv: code cleanup: msr.h ...

- fix code style: mix of tab and white spaces, alignments, etc.
- remove not meaningful comments. Also these comments are below the
  source code which makes the code look messy.
- add more MSR register definitions
