kubeadm: Start to remove old envparams

**What this PR does / why we need it**:

Historically, the envparams feature was used as a way to easily debug `kubeadm` while developing it in the v1.3-v1.4 timeframe. Since then some parameters have been hanging around and not being moved into the API as they should have.

Note: This is a temporary step; moving things into the API. Still, the API is gonna change, this is not the end state. But this is better than keeping the envparams.

I'm gonna deal with `KubernetesDir` in the next PR.

vSphere for cloud-controller-manager

**What this PR does / why we need it**:
This is to implement the `NodeAddressesByProviderID` and `InstanceTypeByProviderID` methods for cloud-controller-manager for vSphere cloud provider.

Currently vSphere cloud provider only supports VMs in the same folder.
Thus `NodeAddressesByProviderID` is similar to `NodeAddresses` with a simple ProviderID to NodeName translation.

`InstanceTypeByProviderID`  returns nil as same as `InstanceType`.

Make storage e2e tests start with [sig-storage] instead of [k8s.io].

This makes understanding sig ownership from a test name very easy for
tools and humans.

- Use a SIGDescribe helper function that adds a [sig-storage] prefix instead of [k8s.io] for tests in storage/
- Move a test that should be in storage into storage.
- Make tests owned by multiple SIGs (configmap test) have [sig-storage] instead of [Volume] labels.

This means that all tests that sig-storage directly owns can be found with a simple regex.

This will be used to make a testgrid dashboard for sig-storage.

Helper methods dealing with ControllerRef

**What this PR does / why we need it**:
Adds helper methods for working with **controllerRef** (controller's `OwnerReference`).

It is based on the existing code from Kubernetes plus extracting some common logic:
- `NewControllerRef` is based on examples from [daemon controller](https://github.com/kubernetes/kubernetes/blob/master/pkg/controller/daemon/daemoncontroller.go#L1223), [deployment controller](https://github.com/kubernetes/kubernetes/blob/master/pkg/controller/deployment/sync.go#L649), [job controller](https://github.com/kubernetes/kubernetes/blob/master/pkg/controller/job/utils.go#L34)
- `GetControllerOf` is copied from [controller_ref_manager.go](https://github.com/kubernetes/kubernetes/blob/master/pkg/controller/controller_ref_manager.go#L36)
- `IsControlledBy` is a common logic extracted from resource controllers: [deployment_util.go#L568](https://github.com/kubernetes/kubernetes/blob/master/pkg/controller/deployment/util/deployment_util.go#L568), [history.go#L276](https://github.com/kubernetes/kubernetes/blob/master/pkg/kubectl/history.go#L276) and many others.

It will also be useful for writing custom resource controllers, for example [service-catalog#979](https://github.com/kubernetes-incubator/service-catalog/pull/979)

"rbd: image xxx is locked by other nodes" is misleading

**What this PR does / why we need it**:

For RWO PV, RBD plugin tries to fence it first, but there are many situations which may cause lock to fail, e.g.

- userSecret is incorrect
- monitor addresses are incorrect or node cannot access ceph cluster temporarily
- image is locked by other nodes
- maybe more...

So, original "image xxx is locked by other nodes" is incorrect in some cases and misleading in diagnosis.

This PR change the error to be correct and not misleading first. We may add detail error descriptions later.

New FailedMount event example if RBD plugin cannot lock image:

Setting default FlexVolume driver directory on COS images.

**What this PR does / why we need it**: The original default FlexVolume driver directory is not writable on COS. A new location is necessary to make FlexVolume work.

This directory doesn't exist by default. FlexVolume users need to create this directory, bind mount it, and remount with the executable permission. The other candidate is /home/kubernetes/bin, but the directory is already getting cluttered. I will submit a different PR for a script that automates this step.

kubeadm: begin polling for bootstrap cluster info immediately

**What this PR does / why we need it**:
This PR changes the behavior of the `kubeadm` loop that polls the API server waiting for discovery information (`cluster-info`). The previous (inadvertent?) behavior was to sleep for `constants.DiscoveryRetryInterval` (5 seconds) before the first request ([`PollInfinite`](https://godoc.org/k8s.io/apimachinery/pkg/util/wait#PollInfinite) vs. [`PollImmediateInfinite`](https://godoc.org/k8s.io/apimachinery/pkg/util/wait#PollImmediateInfinite)).

After this change, `kubeadm` begins does the first check immediately before it moves into the slower polling mode. This takes around 5 seconds off the average time for a new node to join.

#### Before (~5.5s)
