When HV pass through the P2SB MMIO device to pre-launched VM, vgpio
device model traps MMIO access to the GPIO registers within P2SB so
that it can expose virtual IOAPIC pins to the VM in accordance with
the programmed mappings between gsi and vgsi.

Some OSes assume the platform must have the IOAPIC. For example:
Linux Kernel allocates IRQ force from GSI (0 if there's no PIC and IOAPIC) on x86.
And it thinks IRQ 0 is an architecture special IRQ, not for device driver. As a
result, the device driver may goes wrong if the allocated IRQ is 0 for RTVM.
This patch expose vIOAPIC to RTVM with LAPIC passthru even though the RTVM can't
use IOAPIC, it servers as a place holder to fullfil the guest assumption.
After vIOAPIC has exposed to guest unconditionally, the 'ready' field could be
removed since we do vIOAPIC initialization for each guest.

As ACRN prepares to support platforms with multiple IO-APICs,
GSI is a better way to represent physical and virtual INTx interrupt
source.
1) This patch replaces usage of "pin" with "gsi" whereever applicable
across the modules.
2) PIC pin to gsi is trickier and needs to consider the usage of
"Interrupt Source Override" structure in ACPI for the corresponding VM.

Reverts 538ba08c: hv:Add vpin to ptdev entry mapping for vpic/vioapic
ACRN uses an array of size per VM to store ptirq entries against the vIOAPIC pin
and an array of size per VM to store ptirq entries against the vPIC pin.
This is done to speed up "ptirq entry" lookup at runtime for Level triggered
interrupts in API ptirq_intx_ack used on EOI.
This patch switches the lookup API for INTx interrupts to the API,
ptirq_lookup_entry_by_sid
This could add delay to processing EOI for Level triggered interrupts.
Trade-off here is space saved for array/s of size CONFIG_MAX_IOAPIC_LINES with 8 bytes
per data. On a server platform, ACRN needs to emulate multiple vIOAPICs for
SOS VM, same as the number of physical IO-APICs. Thereby ACRN would need around
10 such arrays per VM.
Removes the need of "pic_pin" except for the APIs facing the hypercalls
hcall_set_ptdev_intr_info, hcall_reset_ptdev_intr_info

Now only PCI MSI-X BAR access need dynamic register/unregister. Others don't need
unregister once it's registered. So we don't need to lock the vm level emul_mmio_lock
when we handle the MMIO access. Instead, we could use finer granularity lock in the
handler to ptotest the shared resource.
This patch fixed the dead lock issue when OVMF try to size the BAR size:
Becasue OVMF use ECAM to access the PCI configuration space, it will first hold vm
emul_mmio_lock, then calls vpci_handle_mmconfig_access. While this tries to size a
BAR which is also a MSI-X Table BAR, it will call register_mmio_emulation_handler to
register the MSI-X Table BAR MMIO access handler. This will causes the emul_mmio_lock
dead lock.

When the lapic is passthru, vpic and vioapic cannot be used anymore. In
current code, user can still inject vpic interrupt to Guest OS, this is
not allowed.
This patch remove the vpic and vioapic initiate functions during
creating VM with lapic passthru. But the APIs in vpic and vioapic are
called in many places, for these APIs, follow the below principles:
1. For the APIs which will access uninitiated variables, and may case
hypervisor hang, add @pre to make sure user should call them after vpic or
vioapic is initiated.
2. For the APIs which only return some static value, do noting with them.
3. For the APIs which user will called to inject interrupt, such as
vioapic_set_irqline_lock or vpic_set_irqline, add condition in these
APIs to make sure it only inject interrupt when vpic or vioapic is
initiated. This change is to make sure the vuart or hypercall need not
to care whether lapic is passthru or the vpic and vioapic is initiated
or not.

1) The previous implementaion will recalculate the whole EOI-exit bitmap for
each RTE once the destination, trigger mode, delivery mode or vector of a RTE
has changed and update the EOI-exit bitmap for each vcpu of the VM.
In this patch, only set the corresponding bit of EOI-exit bitmap for
a vcpu when a level triggered interrupt has accepted in IRR or clear the
corresponding bit of EOI-exit bitmap for a vcpu when a dege triggered interrupt
has accepted in IRR which means only update a bit of EOI-exit bitmap in a vcpu
when updating TMR.
2) Rename set eoi_exit related API to set eoi_exit_bitmap.

As we enable Interrupt Remapping, bit positions in IOAPIC RTEs
have a different syntax for programming. ACRN should handle original
format for vIOAPIC as well IR representation for physical IOAPIC.
This patch adds bit granularity IOAPIC RTE.

Under sharing mode, VM0 is identical with SOS VM. But the coupling of
SOS VM and VM 0 is not friendly for partition mode.
This patch is a pure term change of vm0 to sos VM, it does not change
any code logic or senmantic.

This commit changes the EOI_EXIT_BITMAP as follows:
- add a eoi_exit_bitmap to vlapic structure;
- go through all the RTEs and set eoi_exit_bitmap in the vlapic structure when related RTE fields are modified;
- add ACRN_REQUEST_EOI_EXIT_UPDATE, if eoi_exit_bitmap changed, request the corresponding vcpu to write the bitmap
to VMCS.

IEC 61508,ISO 26262 standards highly recommend single-exit rule.
Reduce the count of the "return entries".
Fix the violations which is comply with the cases list below:
1.Function has 2 return entries.
2.The first return entry is used to return the error code of

commit 026ae83bd5 ("hv: include: fix 'Unused procedure parameter'")
removed the then unused parameter handler_private_data from
hv_mem_io_handler_t because MISRA-C requires that there should be no
unused parameters in functions.
This patch removes vcpu from the parameter list as well since this may
not be used by all users. Also it brings back handler_private_data which
is more flexible. For example, vioapic_mmio_access_handler() can use it
to pass vcpu pointer.

a guest may use per-cpu vector allocation mechanism which may lead to
same vector on different vcpu for different vioapic pins.
when we broadcast EOI from vlapic to vioapic, it could trigger
ptdev_intx_ack for incorrect vioapic pin which will lead to incorrect
vioapic irq deassert.
old implementation is recording assert & deassert by acnt, like:
- assert -> acnt++
- deassert -> acnt--
which means an incorrect deassert may descrease acnt from 0 to -1, it leads
to error.
this patch change the recording method for a pin assert/deassert, it
just use pin_state bit set/clear to indicate line state:
- high -> pin_state bitmap set
- low -> pin_state bitmap clear
and the irq assert will be triggered based on polarity setting during line
state changing.
it can avoid the failure of incorrect deassert on old implementation, and for
ptdev sharing irq, the dev hardware could re-trigger interrupt after deassert.
but for emulated device, there is no mechanism to re-trigger interrupt if
there is irq sharing - for DM, we already has constraint to restrict vGSI
sharing, so there is no problem of it.
